My experience...

I only trust one airline to fly my dogs and that is the German airline Lufthansa. I’ve done it several times.

Lufthansa takes excellent care of animals and in case of delays they have a dedicated Animal Lounge in Frankfurt where they take care of all sorts of animals and have a vet on site. There is a flight direct nonstop from Frankfurt - Miami.

A few months ago I flew from Munich to San Francisco, nonstop, with my pup. I bought a Lufthansa x-Large crate for 180Euros. I wanted the large one for him to grow into. Lufthansa sells excellent crates that have no sharp surfaces that can hurt the dog... like sharp plastic window slates as many other crates might have. But they are expensive. You never know if you pup will try with all his might to escape and catch a paw or tooth in the sharp-edged plastic nooks and crannies of cheaper crates. 

Pup in cargo cost around US$500 with me in the plane. If one uses a smaller crate, it may cost less or even count as baggage if you only have one suitcase. If you only ship dog, not sure of the cost but probably the same as I paid. Maybe someone else has more info on that.

I wouldn't use a US carrier if I had a choice.
