I've noticed on some GSD's there appears to be a tendon that slightly bulges out on the outer rear part of the elbow and some dogs are completely flat at the elbow.  I'm wondering if anyone can explain this?

Do you have a photo of what you are talking about? Of course, the anatomy on all GSDs would be the same. Tendons that bulge can be seen more easily and if the dog does not have muscle or other tissue there, you will see a tendon.

There are conditions however that affect the elbow joint and can cause problems. Most of the time it is simply fatty tissue that builds up in that area. But there is a condition that can exist in GSDs and other large breeds where they develop what is called a "hygroma". This condition is fluid in a skin pocket that develops because of inflammation and repeated trauma, such as constantly being on concrete floors for sleeping. Many dogs never have a problem with hard surfaces, while others do.The condition is often not a big deal and will clear up with treatment, and in other cases, can be almost impossible to clear up. I outline the area in red.  she has it on both sides an doesn't seem to effect her.  When I touch the area there's no sensitivity.

This area you have outlined does not look normal. It is difficult to say for sure from a photo like this, but I would suggest that you consider asking your vet about this elbow. Hygromas are not generally painful. It could be just the photo, or it could be tissue, but it looks like it is an area that might also be swollen. What type of surface does the dog spend most of its time on, especially when sleeping?

I pretty sure it's not Hygromas - It's not the skin.  My house is combination harwood floor and carpet and she manages the surfaces pretty well.  She sleeps in the house at night on her pillow, couch or with me in bed.  I don't think her sleeping surface is an issue.  Her crate has two blankets - very cushiony.  She's only 5 1/2 month old.  Her activity level is good and she manages turns an stops well.  When she lays down and extends her legs the area flattens out - no buldge and this is for both sides.  It appears only when she puts pressure on the front legs like when standing.  It's not swelling because it's literally identical on both sides and as I mentioned it recedes when she lays down.  Also, she's not sensitive in the area.  I'll ask the breeder what he thinks on Saturday when I see him again.
