Does anyone know what the process is to get the paperwork from SV to do Hip and Elbo cirtification, the vet told me, I need to get the forms from SV in order for them to fill out and sign with the X-Rays. They said that they can download the OFA forms but not SV forms? I have a 24 month old Import that needs hips and elbos done. any help with how or who to contact will help.

Are you a member of USCA? The forms and instructions are on their website.
